3) Home phone number
4) Your origin: Ashkenazi, Sefardi, both or Convert
5) From which country do your grandparents originate before coming
to the U.S.A

Q: When will the test results be ready?
A: Results of your test will be available in a minimum of 2 weeks
(8 weeks for schools and yeshivos) from the date of testing (Yomim Tovim,
Legal Holidays or specimens coming from out of town may be delayed)

A: Results are only available by contacting Dor Yeshorim at 718-384-6060 You
must call from your home phone number.
